Incident Reporting
Our objective is to ensure that all incidents are reported, investigated and corrective actions implemented to prevent recurrence.
The reporting of incidents and / or near misses is an integral component of the Health, Safety, Environment and Quality (HSEQ) Management System. Effective reporting of incidents and near misses, allows for deficiencies within the system to be identified, enabling corrective actions to be implemented in order to eliminate and / or reduce the identified risk to as low as reasonably practicable.
Team Member Injury
In the event of a team member injury, incident reporting procedures must be followed but the provision of immediate first aid attention to our recovering team member is the highest priority.
Breach or Failure to Report an Incident
All team members have a duty of care and responsibility to report incidents as soon as they occur. Failure to do so may result in disciplinary procedures being initiated.
Standard Reporting Process
It is the responsibility of all Company team members to report incidents and / or near misses to their Supervisor and / or Leader as soon as they occur.
Upon notification, it is the responsibility of the Site Manager to report the incident on CHESS or via the Compass Incident and Emergency Line.
